Not to Change, but to Restore: How Dr. Yoko Nakano Plans Fillers, Botox, HIFU, and M-Tight in Tokyo

In short

At Art Lounge Clinic in Azabudai, near Roppongi, clinic director Dr. Yoko Nakano plans hyaluronic acid fillers, botulinum toxin, HIFU with the New Doublo, radiofrequency (Hydro Phase), and M-Tight around one idea: not to create a new face, but to restore the balance each person already has. She assesses the layers of the face, left–right differences, and expression before deciding on treatment, and uses ultrasound to confirm the plane for fillers and the position of the SMAS for HIFU. Hyaluronic acid filler is ¥104,500 per 1 cc, Botox is ¥22,000 per area, a full-face HIFU session is ¥96,000 to ¥199,600, and M-Tight is ¥100,000 for two areas, tax included. Dr. Nakano offers consultations in English by appointment.

01

What do you mean by "not to change, but to restore"?

In aesthetic medicine, my goal is not to create a new face, but to restore the balance each person already has.

Before deciding what to do, I first ask where the face has changed, and why. That is where aesthetic medicine that restores, rather than changes, begins.

02

How does the face age, and what do you look at before deciding on treatment?

The face ages layer by layer: skin, fat, SMAS, muscle, and bone each change in their own way (Rohrich & Pessa, 2007; Mendelson & Wong, 2012). That is why I assess the layers, left–right differences, and expression before deciding on treatment.

At the first consultation, I look at which layer has changed, by how much, and on which side, and I ask what you would like to change and what you would like to keep. The treatment plan follows from that assessment, not the other way around.

03

How do you use hyaluronic acid fillers?

Rather than simply adding volume, I place small amounts where structural support has been lost. I use ultrasound to confirm the plane and avoid vessels.

04

What is your approach to botulinum toxin?

The aim is not a frozen face. I release only excess tension, such as the downward pull of the platysma, while preserving natural expression (Fabi et al., 2025).

05

How do you use HIFU and radiofrequency?

HIFU targets the SMAS, whose depth varies between individuals, so I confirm its position with ultrasound. I combine it with radiofrequency (Hydro Phase) to improve skin quality.

06

What is M-Tight, and what does the evidence say?

Facial muscles thin with age (Okuda et al., 2023). Early studies suggest that electrical muscle stimulation can increase facial muscle thickness and refine contours (Okuda et al., 2026; Frank et al., 2025). These studies used other devices and were small, and data on M-Tight itself are not yet published. We will evaluate outcomes carefully with ultrasound.

07

Why combine treatments?

Expert consensus recommends combining treatments across layers, based on individual assessment (Carruthers et al., 2016). The Canada HARMONY Study showed that panfacial treatment improved not only satisfaction with appearance, but also psychological well-being (Rivers et al., 2025).

Results vary between individuals.
